· Medication for individuals with a diagnosis of kidney disease and financial need may apply for assistance. Participants may receive a maximum of $400 in a calendar year.
· Transportation for individuals with a diagnosis of kidney disease and financial need may apply for financial aid. Participants must travel a minimum of 20 roundtrip miles to dialysis or transplant appointments to qualify. If approved, applicants receive a monthly reimbursement of $35 by check.
· Emergency help for individuals with a diagnosis of kidney disease and financial need by apply for financial aid for a maximum of $100 to be paid for emergencies such as car repairs or medical supplies.
Education:
· Provides a wide variety of educational materials addressing prevention and treatment of kidney disease. Staff are also available to answer questions or provide support.

· Volunteers from St. Mary's Catholic Church, Wakeman Congregational UCC & Clarksfield Methodist provide assistance to qualified families who reside in Wakeman, Collins, Hartland, Clarksfield and Birmingham:
· Donated funds & food support a monthly food distribution out of St. Mary's Hall the 4th Tuesday of each month. Clients must meet USDA income requirements and not be receiving food from other food banks.
· Emergency Assistance is available for families facing eviction or receiving a disconnect notice from an electric or water utility. Applications for assistance must be made in person at St. Mary's Office during 9am-1pm Monday thru Friday. Bring the eviction letter or disconnect notice. Amounts granted are limited.
· A Senior Luncheon is held the 3rd Wednesday each month at St. Mary's Hall. A $2 minimum is requested to cover costs. All are invited. Call St. Mary's several days in advance to reserve a lunch.
